The dollar amount you must pay in healthcare costs before your insurance plan begins to contribute.


A list of prescription drugs covered by an insurance plan. Most formularies list prescriptions in various tiers that represent levels of coverage or cost. Some Medicare plans restrict doctors to prescribing only the drugs listed in the formulary.

Out-of-pocket costs

The healthcare costs you pay that aren’t reimbursed. Out-of-pocket costs include copays, premiums, deductibles and coinsurance payments.


Fees levied for missing Medicare’s initial enrollment period or for not carrying prescription drug coverage. These fees may be added to your monthly premium for as long as you have Medicare coverage.


A set dollar amount you must pay each month to maintain your insurance coverage. This price may change each year. Some Medicare Advantage Plans have $0 premiums.
